Can’t afford a gym membership? Submit a short video — 2 to 3 minutes, filmed on any phone — telling us who you are, which registered FSF partner gym you want to attend, and why you want this scholarship.

If we have the funds, we’ll pay for your first full year of training at your chosen gym.

📋 Media Release & Permission Agreement

I, the undersigned parent or legal guardian of the scholarship applicant named in this form, hereby voluntarily grant FightState Foundation, a Florida nonprofit corporation, the irrevocable right and permission to use, reproduce, publish, display, distribute, and share the scholarship application video and any photographs, images, name, likeness, voice, and personal story submitted by or on behalf of my child/ward. This permission extends to use across FightState Foundation’s social media platforms (including but not limited to Instagram, Facebook, YouTube, TikTok, and X/Twitter), website, email communications, fundraising campaigns, and promotional materials. I understand that: (1) All submitted videos are reviewed privately and require explicit FSF approval before any public sharing; (2) This content may be shared publicly after approval; (3) FightState Foundation will present my child’s story with dignity and respect at all times; (4) This permission is granted freely and voluntarily, without compensation; (5) Submission does not guarantee a scholarship; (6) I may request removal of my child’s content at any time by emailing info@fightstatefoundation.org.

Volunteer With Us

FightState Foundation relies on volunteers for application reviewing, outreach, social media, gym relations, and fundraising. Must be 18+, willing to complete a background check.
